  What is the Wellness Benefit?

• The Aflac Accident plan has a Wellness benefit each year, which is a $60 annual reward for anything like a dental visit, eye visit, physical, mammogram, etc.  We all care about you and want to thank you for doing something preventive. On something easy like a Wellness claim, you don't even need to provide proof.  It's the honor system. (Bigger injuries need the doctor notes and proof)   This is a $60 payout once per family per year (not per person covered).  
   
• Some Cancer plans have a Wellness benefit as well each year, which is an annual reward for anything "screening-ish" like a chest xray, mammogram,  colonoscopy, Well Woman, PSA, biopsy, bloodwork, etc.  Being proactive with screenings saves everyone treatment and money down the road, so Aflac wants to thank you for those screenings.   Like Accident policies, for a Wellness claim, you don't even need to provide proof.  (Bigger diagnosis' like internal cancers need the doctor notes/ pathology reports as proof)  This is a reward that pays out once per covered person per year.

• Find your policy number.  Please note that there are no letter "O's", only the number "0".
• If you haven't logged in and registered, go to www.aflac.com/myaflac or search for the MyAflac app on your phone. When you first Register/ Create a Logon, it will ask for a policy #... just  paste yours in there.  Once it verifies you are a policyholder, you'll soon create a name and password, and finish the steps.   
• Once you are in:  you will quickly see ways to view and print your policy... as well as update your address and also set up Direct Deposit.  Of course, there is a large link to "File a New Claim" and it guides you through some easy questions.   
• When you file a "Wellness" claim  online, simply look for the option  "I had routine and preventive services", and be ready to provide the Dr, the date, and what kind of visit.  Done!
  • If you have something actually happen (breaks, burns, stitches, ER visit, etc), that's when the actual documentation needs to be attached... it's your “proof” of diagnosis and treatment.

If you are unable to complete the registration steps, possible causes are:

• There are no letter "O's" in a policy number, only the number "0".
• If your name is hyphenated, try removing the symbol.
• Contact Aflac or Dave to re-review your personal info. There may be an old address, or simply a typo in the email, DOB, or zip code somewhere in the system.
